Beacon Health Systems

Beacon offers a wide variety of health services throughout the Michiana area including Memorial Hospital located in downtown South Bend. They also manage three Neighborhood Health Centers, which accept Medicaid, Medicare and private insurance, and offer a sliding fee scale based on income guidelines for uninsured. They participate in Hoosier Healthwise, a program that provides health care for Indiana children, pregnant women and low-income families and also offer a special cash discount program for patients who do not have insurance or qualify for other assistance. First Steps

The goal of First Steps is to help Hoosier families make sure their infants and toddlers receive services now to help them in the future. Services through First Steps are designed to meet the developmental needs of the child and address any delays that may be occurring. First Steps serves the following counties: St. Joseph, Elkhart, LaGrange, Steuben, Noble, DeKalb, Whitley, Marshall, Fulton, and Kosciusko.

HealthLinc Community Health Centers

HealthLinc provides comprehensive health care at two Community Health Centers in St. Joseph County. The comprehensive health care includes: medical care for people of all ages, mental health care resources, vision care, dental care, pharmacy assistance and community outreach. HealthLinc offers health insurance enrollment assistance for Federal Health Insurance Marketplace, Indiana HIP 2.0 and Medicaid assistance. Their Community Health Centers are located at:

The Navari Student Outreach Clinic (NSOC)

Basic health screenings, health physicals, chronic disease consultation and management, STD screening, pregnancy testing, basic laboratory testing, minor acute care during clinic hours (e.g. splinting, suturing, etc.), limited pharmaceutical dispensary, referrals to other medical practices within the community, social services support, periodical health events. The clinic serves all ages, from infants to senior citizens. Anyone is welcome to seek treatment there, although it’s primarily designed to serve area residents who lack health insurance or access to regular medical care. St Joseph Health System – Community Health Centers

These centers have been established to serve the uninsured, underinsured, and Medicaid populations. Our centers offer traditional healthcare services and medication assistance programs for those patients who qualify. All patients are screened for financial assistance. Those patients who qualify will be offered benefits assistance from Medicaid and the various other local, state and federal aid programs. There are three Community Health Centers in St. Joseph County:

Provides a ‘WIC Card’ that can be used at local grocery stores to purchase milk, cereal, yogurt, juice, fruit, vegetables, etc. Also provides formula for mothers who are not able to breastfeed. There are two WIC locations in the Michiana area.

Center for Healing and Hope

An urgent care center for the uninsured, the under-insured, and those with unmet health care needs in our community. We function as a point of entry for those with no access to our health care system. Our patient advocate refers as many people as we can to primary care physicians, including Heart City and Maple City clinics. Note: We do not provide primary care to patients, but refer them to clinics that do. Our care goes far beyond treating injuries and illnesses. We also connect families to resources such as housing, food, and counseling services.

The Findley Dental Center – Heart City Health

Heart City Health Center’s licensed dentists and hygienists provide general dentistry services to people of all ages, incomes, and backgrounds. They also accept Medicare/Medicaid.

Give Back a Smile Program Program of American Academy of Cosmetic Dentistry Charitable Foundation

Provides restorative and cosmetic dentistry in the smile-zone to qualified survivors of domestic violence. Eligibility: $20 donation or proof of 10 hours of community service. Requires referral from a case manager or social worker.

Heart City Health Center

At Heart City Health we take a multifaceted approach to healthcare that allows us to provide care across multiple disciplines, to treat the whole person. Our services include family medicine, dentistry, behavioral health, and a full-service pharmacy all under one roof. Assistance is also provided to guide you through the insurance process so you have the coverage you need. There are 2 locations for the Health Centers:

Northern Indiana Hispanic Health Coalition

In collaboration with numerous local agencies, foundations, schools, and volunteers, the NIHHC provides a variety of beneficial services: health care intervention programs, health fairs providing free health screenings, referrals to physicians and health care facilities that are attuned to the Hispanic community’s needs and advocacy for language interpretation and health insurance needs
